Map header updates: Correct creation date in map header that was changed in r7448, it was using the wrong month; Update maps with entrance x & y coordinates, make map name more descriptive, adjust map level difficulty to something slightly more realistic

Resize some mlab maps that were mostly a lot of empty space, in some a few things outside map (like buttons/boulders) had to be moved a bit. In a few these things outside the map were actually not used.
